We ask that you please sign up for our program before using our services. The process can be started by following the steps below.

Please note that you are required to sign up BEFORE your first visit to the Cougar Cupboard.

How to sign-up section

1. Use this link to complete your registration. It is a 3-step form, and you must complete all three sections to complete registration. (QR code is also attached)
   1. First part of the form - demographic information for Cougar Cupboard Data Collection
   2. Second part of the form - answer questions to make sure that you understand the services you will receive at the Cupboard.
   3. Third part of the form - Houston Food Bank questionnaire. Once you complete this section, you will receive an FSH number. Please copy and paste that ID (including the FSH portion) to the original form.
   4. Submit and you are all done!
2. When you go to the Cupboard for the first time, please share your FSH ID. You will receive an ID card with your name, FSH ID, and PSID. Treat this card like a debit card! You will need it every time you come into the Cupboard.

The Houston Food Bank is not a government service and does not receive federal funding. Client information does not leave the Houston Food Bank and is only used for research purposes. Individual client data or identifying information will not be shared.
