MSW Advising Forms

Most GCSW forms are available on Office 365. To access the forms, first login to your Office 365 account by clicking the Office 365 icon in your myUH account. Then click the link for the form you want to complete. Information about logging in to your Office 365 can be found here.

Degree Plan Checklist

This checklist, which can be used for any MSW enrollment model and concentration, can serve as a good tool to keep you on track with your degree plan.

Request for Leave of Absence (LOA)

If you need to take a leave of absence from the GCSW, please complete this form. Your advisor will follow up with further instructions as needed. If you need to drop classes or withdraw, you may also need to complete university forms. Please refer to the UH Graduate School Forms section below for more information.

Request for Change in Enrollment Model
To request a change in enrollment models, please complete this form prior to registration in order to be considered for the next semester. Your advisor will follow up with you.
Request to Take Courses Outside the GCSW

If you would like to take a course outside the GCSW for an open elective credit, please complete this form. If you are requesting a course outside the GCSW, you may need to submit a copy of the syllabus of the course you are interested in taking. Please note that only graduate level courses will be considered as a possibility for open elective credit.

Focused Learning Opportunities Interest Form

Focused Learning Opportunities (FLOs) are optional ways students may develop an in-depth and holistic focus in a specific area of social work practice. FLOs are organized around areas of expertise of current GCSW faculty, enabling students to pair learning in a variety of environments (classroom, practicum, community engagement, trainings, etc.) with meaningful mentorship from GCSW faculty members. Independent Study Policy & Procedures - MSW Program

An Independent Study provides students the opportunity to work with a faculty member on a specific topic or issue of interest that might not otherwise be available in a structured course. Independent study courses are not available to students until they have completed the foundation curriculum. Independent study courses count toward the degree as an elective. Student Initiated Drop Form

If you need to withdraw from a class after the twelfth day of classes or 4th class day for Sessions 3 & 5 and Summer Sessions , you must gain your instructor's signature to drop a class. This form must be submitted to the Office of the University Registrar by the close of business of the last day to drop a class. We also ask that you notify your advisor. On or before the twelfth day of classes (or 4th class day for Sessions 3 & 5 or Summer Sessions), students have access to drop classes online through myUH.

Student Initiated Term Withdrawal Form

After the first day of classes each semester, students wishing to drop all of their classes for the semester must complete this form to request a full term withdrawal. Additionally, if this request occurs after the twelfth day of classes (or 4th class day for Sessions 3 & 5), you must also submit a student-initiated drop form for each class.  Also, international students have to attach a completed and approved Reduced Course Load form with this request. Please notify your MSW program advisor. This form is used when a student is withdrawing from a term for documented medical or administrative reasons (including military deployment and death of an immediate family member). Appropriate documentation should be attached to the request. Withdrawal paperwork should be submitted to your advisor to begin the approval/disapproval process.

Student Employment Forms

GCSW Graduate Student Employment Application

Assistantships are available to students admitted unconditionally to the MSW Program. Once applicants have received their acceptance letter for admission to the MSW program, they may apply for an assistantship. Send a resume with this form to Ann Liberman at aliberman@uh.edu.
