Become A Preceptor
To be eligible to act as a preceptor, one must maintain the requirements set forth by the Texas State Board of Pharmacy before completing the steps below to become a preceptor at University of Houston College of Pharmacy.

Preceptor Application
We are excited to hear that you are interested in becoming a UHCOP Preceptor. To assist you in the process, we have outlined the university requirements below.
Step 1: Demographics Survey
Please complete the survey at the link provided. This will give us greater insight into your area of practice and determine if your organization has a site coordinator responsible for rotation assignments. Start Preceptor Demographics Survey
Step 2: Recommendation Form
Upon completion of the survey, download the Preceptor Recommendation Form. The form must be completed on your behalf by a fellow pharmacist and submitted along with your CV/Resume. All forms can be submitted by email: uhcopexp@uh.edu.
Step 3: Curriculum Vitae/Resume
Please submit a copy of CV/Resume along with your recommendation form. All forms can be submitted by email: uhcopexp@uh.edu.
Step 4: Review
Upon successful submission of the required documents, your profile will be submitted for review and approval. If you have any questions regarding becoming a UH preceptor, please contact the Experiential Programs Office at uhcopexp@uh.edu or 832-842-8337.
Step 5: Notification
You will be notified of your approval and will be provided pertinent information and
documents.
